Learning Objectives
Upon completion of this activity, participants should be able to:

  1. Recognize the importance of addressing Social Determinants of Health
  2. Demonstrate the ability to assess for patient’s need.
  3. Develop a plan designed for the patient’s home environment when discharged.
  4. Examine and Identify community resources through FindHelp.
  5. Utilize software to communicate resources and referrals to the patient.
  6. Document intervention and recommendations in patient’s electronic health record.
