The Somatic Symptom Disorders section of the DSM was substantially revised for DSM-5 in 2013.  In the decade since their redefinitions, the core concept that somatic symptom and functional disorders must be defined by the presence of identifiable symptoms and signs has made inroads into medicine, partially displacing the outmoded idea of “medically unexplained symptoms” as a diagnostic concept.  Nevertheless, crucial questions about these disorders have not been answered.  This presentation will: (1) describe the rationale for moving away from historical concepts of somatization, hypochondriasis, and conversion disorder, (2) explain the clinical features of somatic symptom burden, illness-related anxiety, beliefs, and behaviors, and manifestations of functional neurologic disorder, and (3) discuss unanswered questions and controversies such as, “Is somatic symptom disorder a useful concept or a relic of a bygone era?” “Is it a single entity or a kaleidoscope of clinical syndromes?”  “Why is functional neurological disorder in the psychiatric nomenclature?” “Is central sensitization the underlying mechanism of somatization or a biological metaphor for the psychosocial process of illness-related anxiety, beliefs, and behaviors?”
