DESCRIPTION
Primary Care Grand Rounds at the Mayo Clinic Health System in Northwest Wisconsin is a monthly educational series centered on improving care and innovating healthcare delivery in primary care practices. Sessions are held the first Friday of each month from 7-8 am at the Auditorium of the Luther Building in the Mayo Clinic Health System, Eau Claire Hospital and via Zoom

TARGET AUDIENCE
This course is intended primarily for physicians, nurse practitioners, physician assistants, and administrators in primary care (Family Medicine, Internal Medicine, Pediatrics, Urgent Care)

LEARNING OBJECTIVES
Upon conclusion of this activity, participants should be able to:

•    Explain how to assess their ability to coordinate team-based care and improve systems.
•    Describe how to provide consistent patient-centered care that improves the patients’ experiences with their providers.
•    Explain how to apply team-based care concepts for optimal care for patients with chronic conditions. 
•    Describe how to provide cost-conscious medical care by decreasing low acuity emergency department visits and hospital readmissions.
