Advanced Care at Home (ACH) spans the transition from inpatient to post acute care. Currently the literature does not necessarily cover this delivery of care in-depth.
Advanced Care at Home Grand Rounds will explore topics pertinent to our patient population as the move from inpatient level of care to post acute care management and considerations for applicability of current body of literature to our patients. The series will meet monthly, and be open to all providers of at-home care including physicians, mid-level providers and pharmacists.
