DESCRIPTION
Primary Care Grand Rounds at the Mayo Clinic Health System in Northwest Wisconsin is a monthly educational series centered on improving care and innovating healthcare delivery in primary care practices.

In-Person and Live-Streamed; Monthly on the 1st Friday from 7:00-8:00 a.m.

TARGET AUDIENCE
Allied Health Professionals, Healthcare Executives, Nurse Practitioners, Pharmacists, Physicians, and Physician Assistants.

LEARNING OBJECTIVES
Upon conclusion of this activity, participants should be able to:

  • Assess clinical staff's ability to coordinate team-based care and improve systems.
  • Apply team-based care concepts for optimal care for patients with chronic conditions. 
  • Describe how to provide cost-conscious medical care by decreasing low acuity emergency department visits and hospital readmissions.
