Real World Clinical Practice Among Patients with Bipolar Disorder and Chronic Kidney Disease on Long-Term Lithium Therapy
Rakesh Kumar, M.B.B.S., M.D.

Dr. Rakesh Kumar graduated with an MBBS degree from Goa Medical College, Goa, India. He completed his post-graduation training in Psychiatry from Guru Gobind Singh Medical College, Punjab, India. Before coming to Mayo Clinic, he was a practicing psychiatrist at the Guru Gobind Singh Medical College, Faridkot, India. During his training, he was appointed as chief resident in his final year of residency. He also served as a Medical officer and served Govt. of India for almost 07 years. He also worked in Asia’s largest correctional facility at Tihar Jail, New Delhi. His research interests are in understanding the neurological underpinnings of mood disorders and the development of objective biomarkers to aid the assessment and treatment of psychiatric disorders. His main interests are neuromodulation and psychopharmacology. He mainly focuses on adopting new technologies to advance the practice of psychiatry. This includes brain stimulation interventions, biomarker work to optimize diagnostic practices, psychopharmacology, and applications of artificial intelligence (AI) platforms.

Learning Objectives
Upon conclusion of this activity, participants should be able to:

  • identify differences in clinical characteristics among patients with bipolar disorder (BD) and renal insufficiency treated with long-term (≥ 1year) lithium (Li) therapy (LTLT) and its outcomes.
  • identify the real-world clinical practice among patients with BD and chronic kidney disease on LTLT in a two-year mirror image study
  • identify the differences in the real-world clinical practice and outcomes among BD patients with CKD on LTLT who continued vs discontinued Li. .

Treatment-Resistant Bipolar Disorder:  Exposure to Pharmacotherapy
Jorge Sanchez Ruiz, M.D., M.S.

Dr. Jorge A Sanchez Ruiz is a Mood Fellow here at Mayo Clinic. He was born and raised in Monterrey, Mexico. From 2009 to 2021, Jorge studied at the Autonomous University of Nuevo Leon (UANL) in Mexico, where he earned his medical degree, held several research positions, and completed his psychiatry residency. He published his master’s thesis on candidate gene expression in Major Depressive Disorder. His research interests involve mood disorders, translational research, and individualized medicine in psychiatry. Jorge is also passionate about patient education, shared decision-making, and cultural competence in psychiatry.

Learning Objectives
Upon conclusion of this activity, participants should be able to:

  • Describe three comprehensive clinical staging models for Bipolar Disorder and the lack of consensus in defining Treatment-Resistant Bipolar Disorder.
  • Contrast Treatment-Resistant Bipolar Disorder with Treatment-Resistant Depression in the context of their respective goals of treatment.
  • Recognize the association of increasing polypharmacy with worse clinical outcomes in Major Depressive Disorder and Bipolar Disorder, and the implications for defining treatment-resistance.
