DESCRIPTION
In this monthly series, a multidisciplinary team of presenters review an assigned case, and present a systematic analysis of what factors contributed to the adverse/undesired patient outcome. Literature review and data is then presented to discuss what changes could prevent medical errors and system breakdowns in the future. Finally, attendees collaborate and propose practice changes for future implementation.
LEARNING OBJECTIVES
Upon completion of this activity, participants should be able to:
- Explain how systems/process failures/cognitive failures can result in undesired medical outcomes.
- Explain how to implement system-based changes that reduce the incidence of undesired medical outcomes.
