Digital Referral Partners: Virtual Options to Support Patient Needs

LEARNING OBJECTIVES: Upon conclusion of this activity, participants should be able to:
- Describe the clinical rationale and evidence supporting referrals to digital-first care models, including key patient populations who benefit most.
- Attendees will be able to articulate why digital-first care matters, cite outcomes data, and identify which patients are most likely to benefit from these approaches.
- Explain how digital-first referral pathways can be integrated into clinical workflows, including best practices and strategies to overcome common barriers.
- Participants will learn practical steps for EHR integration, provider handoffs, patient self-enrollment, and communication strategies, as well as solutions for challenges like clinician skepticism and digital literacy.
- Evaluate the impact of digital-first referrals using key metrics and feedback loops, and discuss lessons learned from real-world case studies.
- Learners will be able to identify relevant engagement and outcome metrics, understand how feedback is shared between providers, and apply insights from successful implementations to their own practice.
