These updates can also be beneficial to advanced practice professionals working in internal medicine practices.

03/15/2022 - 8:00am to 9:00amSummary: IW is a 50 y.o. female with history of ER/PR positive HER2 negative right breast cancer with metastases to the bone and liver complicated by recurrent malignant pericardial effusions s/p pericardial window as well as recurrent bilateral malignant pleural effusions s/p placement of bilateral PleurX catheters. She was hospitalized for respiratory distress in the setting of worsening pericardial effusion. Discussion between Cardiology and Cardiothoracic Surgery determined that the risks of an additional pericardiocentesis and/or surgical intervention outweighed the benefits. Palliative Medicine was consulted for assistance with goals of care, with patient ultimately opting to enroll in hospice and be transferred to an inpatient hospice unit. Unfortunately, upon transfer to hospice IPU, the patient was admitted with incorrect medication orders, causing significant distress for both the patient and her husband. We will be performing a root cause analysis to examine the factors contributing to this error. We will also explore how this error was addressed, as well as what we might do better to help facilitate transfers from the hospital to the inpatient hospice unit, specifically in relation to the Palliative Medicine Fellows QI Project. We will also examine what the literature has shown what the most common complaints made by patients and families are when they enroll into hospice.
